The purpose of this questionnaire is to learn more about fatigue and physical activity in children and adults who have juvenile dermatomyositis (JDM). Filling out this questionnaire will help us better understand differences between adults with JDM and adults like you who do not have JDM.
This questionnaire should take approximately 30 minutes to complete. If you do not have 30 minutes to complete the questionnaire right now, please bookmark or copy this website address so you can return at a later time when you have enough time to complete the questionnaire in one sitting. Please do your best to answer every question.
If you need to leave the questionnaire before you are finished, it is possible to save your progress and return to finish the questionnaire at a later time. To save your progress and return at a later time please do the following:
- Click on the "Save & Return Later" button at the bottom of any of the questionnaire screens.
- Follow the directions on the website that will prompt you to:
- Write down the “return code”, a series of numbers and letters which will appear on the screen (you will need it later to return to your saved spot on the questionnaire).
- Bookmark or save the questionnaire website address so you can return to the questionnaire website when you are ready to resume the questionnaire. You may also email the study team at jdm@gwu.edu for a copy of the questionnaire link (although we will not have access to your unique return code).
- OR, enter your email address if you would like to have a link to the questionnaire website emailed to you. Please note, this function will NOT save your email address and it will NOT send you your unique return code.
- When you are ready to return and finish the survey, use the website link you saved or had emailed to you to return to the questionnaire website.
- On the questionnaire page, click on "Returning?" at the top right-hand side of the screen and then click "Continue the survey".
- Enter your return code and the website will take you back to the spot on the questionnaire where you left off.
